In my opinion Motherboard issue, but I can't assure, it could also be the Power supply, as bad power supply could cause the motherboard mal-function and is causing the hard drive failure. If the motherboard is under warranty, you better get a replacement than wasting another Hard drive, and I hope it's better for you to replace the power supply as well, you'll spend less buying a 450watt power supply than buying a new 500gb hard disk. Also make sure the cabinet is properly fitted and the ground is working without any problem, you can check this by using a tester (screw driver tester) after it's connected to the mains, or you can try with your hand (it'll not kill you:-], but I don't recommend if you have a screwdriver with tester).
